This survey was created to get input on a reminiscence program being created for Clark residents to be able to have positive interactions with family members and staff by talking about their life experiences. Your input regarding what you'd like to know from the residents, what stories your loved ones enjoy telling, and what topics to avoid as well as any other suggestions are welcomed and extremely appreciated. Please take a few minutes to fill out this survey as soon as you are able to and prior to 6/19/20 as the survey will stop accepting responses after that date.

All survey responses will be anonymously reviewed by Life Enrichment Staff and survey responses will be used to assist in creating and formatting the reminiscence program. Survey responses will not be distributed or shared in any way outside of the Clark community, and your participation in the survey communicates your agreement with these guidelines and the use of your responses in this manner. If you do not wish for your responses to be utilized in the creation of this program, please do not complete the survey or submit your survey responses.
