If you know that your child will not be able to attend a rehearsal or performance due to illness, church or school functions or family obligations, please notify NDCC as soon as possible by submitting this form.
**Please note that if your child has ANY COVID symptoms, they should NOT ATTEND choir rehearsals. We appreciate your following this policy. Email us at ndcchoir@nd.edu or call us at 574-631-3384 if your child is diagnosed with COVID. If a child displays symptoms at rehearsal, they will be sent home immediately. Thank you for keeping us informed of any necessary absences.
