This form is to apply for Moderator. The application is for both Twitch and Discord.
If you want to be just Twitch Mod, you will have the mod role in Discord as well, but you're not required to so any work if you don't want to. BUT, if you want to just Moderate Discord and can't catch streams, you won't get the Twitch role, but you get the Discord role either way.)
