Thank you for submitting a 60-90 second video to HUB Cycling's 3rd Annual Bike Reels Student Video Contest!

If you created your film with a group, please have ONE person submit this form and make sure to list the full names of anyone else who appears in the video.

To participate in the contest, you must have a parent/guardian complete the video waiver at the end of this form. Everyone who appears in the video must complete the waiver.

The deadline to submit this form and complete your video waiver is SUNDAY, JUNE 6TH. If we do not receive waivers from everyone who appears in the video, your film will not be entered into the contest.

How to submit your film:
☆ After completing this submission form, send your video file to btsw@bikehub.ca via www.WeTransfer.com.
☆ WeTransfer is a free file-sharing service, and you do not need to create an account to use it.
☆ Simply add your video file, email to btsw@bikehub.ca, enter your email address, and put the title of your film in the message. You will receive a confirmation email once it has been sent.

If you don't have your own email address or don't check your email often, please use a parent/guardian's email to complete this form.
