Registration form for First Baptist Church Vacation Bible School July 29th - August 2nd, 6:00 - 8:30.  There will be a small meal served starting at 5:30 that your child can also attend before the VBS begins.  Please fill out a form for each of your children that will be attending.   When filling out the form for multiple children please be sure to fill out the following parts so you do not have to fill out the whole form: In the Child's Name type your child's name and then (same as ____) place the 1st child's name in the (____), gender, age, date of birth, last graded completed, shirt size, allergies, and did a friend invite you.

Shirt Size- Shirts will cost $10.  Your money and order has to be turned in before July 18th in order to guarantee that you receive a shirt.   The shirt order will be placed on July 19th.  You can turn your money at the First Baptist Church, 801 Brown, you could click on this link    shorturl.at/aopDW (be sure to specify that it is for a VBS shirt in the area that says designated, if special fund desired), you could give your money to a member of the church to bring, or you could mail your money to the church (if you mail it it must be to the church before or on July 18th in order for your shirt to be ordered). *
