SB Humane Society: Dog Training Registration
Thank you for your interest in our dog training classes! Please complete the registration form information below; this helps the trainers know what your and your dog's needs and goals are for these classes. Completing this form is the first of 3 steps to participate in the classes:

1. Submit vaccination records and signed waiver to christine@sbhumanesociety.org
2. Complete the form below
3. Submit payment (cash, check or credit card) either over the phone or in person at our Shelter Office during our hours of operation
4. Attend Introduction to Dog Training - required to participate in the Refined Rover class; Reactive Rover has its own orientation
