Scuba VBS 2024 - VOLUNTEER REGISTRATION
VBS is June 3-7, 2024 from 9am-Noon.  We need youth and adult volunteers to help us make our VBS successful! We ask that all youth volunteers will have at least completed 5th grade by the time of VBS.

A separate form must be completed for EACH volunteer. 
Volunteers 18 and over are subject to background checks.

Liability Policy
I, as an adult or legal guardian of the minor child listed within this registration, have given my consent for myself/him/her to volunteer for Vacation Bible School at King of Kings Lutheran Church. I understand that there are risks involved in this event and hereby release the Church, its staff, and volunteer workers from any liability for any injury, loss, or damage to person or property that may occur during the course of this event. In the event that myself or any of the minor children listed within are injured and require medical attention, I consent to any reasonable medical treatment as deemed necessary.

In the event treatment is required from a physician and/or hospital personnel designated by the church, I agree to hold such person free and harmless of any claims, demands, or suits for damages arising from the giving of such consent. I also acknowledge that I will be ultimately responsible for the cost of any medical care should the cost of that care not be reimbursed by my health insurance provider.

Media Policy
During the week of VBS, we will be taking pictures/video of the volunteers and children participating and could post said pictures/videos on King of Kings' social media, online church service, website, bulletin boards, or advertising materials. Pictures/video will be for the use of King of Kings VBS program. Our opening and closing celebration could be live-streamed to Facebook, YouTube, and our church website. No names or ages will be associated with any posted picture or video.
