Thank you for organising your own placement.  We will contact you and the contact person to exchange further information as the work experience week approaches.  This assumes that the contact person named in the form HAS AGREED offering you a place for the week.  Please only submit the form if you HAVE OBTAINED APPROVAL from the company.  Please be reminded that this form must be completed with your parents' consent.
