Contest Details: Welcome to Kingwood Town Center Park's Dog Costume Contest! We're excited to see all the creative and adorable costumes you and your furry friends come up with. Please read the rules and regulations carefully before filling out the form. Only the first 20 contestants will be admitted, or registration will end at noon on 9/13/2023.

• Best Pet/Owner Combo Costume

Prizes:

• Winner of each category: [Prize for Most Creative Costume] and [Prize for Best Pet/Owner Combo Costume]
• Runner-up of each category: [Prize for Most Creative Costume] and [Prize for Best Pet/Owner Combo Costume]

Rules and Regulations:

1. All entrants must gather next to the stage at least 15 minutes prior to the start of the contest. Contest will begin on Saturday, October 14.
2. Only one pet per entry is allowed.
3. Each pet must be on a leash.
4. Pets must be 4 months or older.
5. Entrants must fill out this form online with their name, contact information, and a clear description of their pet and the costume. This information will be used by the MC for announcements and winner publicity.
6. No entrant is eligible for more than one award.
7. Entrants will be given a participant number, which must be worn visibly for judges to see.
8. No motorized vehicles are allowed.
9. Each pet must have its own adult walker.
10. Judges' decisions are final and binding.
11. You must be present to win.

By submitting this form, I acknowledge that I have read and understood the rules and regulations of the Dog Costume Contest. I agree to abide by these rules and understand that judges' decisions are final. I also grant permission for my pet's name, costume description, and my own name to be used for announcements and winner publicity.
