Application Instructions

Applications must be filled out completely. Please note that applying does not guarantee an interview or a spot in the Pre-Apprenticeship program. The priority deadline is December 15, 2022. The main application deadline is February 3, 2023.

All information in this application will be used by Chef Ann Foundation staff to better understand each potential Pre-Apprentice's abilities, situation, and needs. Information will be kept confidential and will not be shared beyond work sites (K-12 districts) and the employer of record (Career Catalyst). Eligibility Requirements

Please review the schedule and time commitment. Then, check each line to indicate that you know and meet the eligibility requirements.

I am 18 years or older (or will turn 18 by March 1, 2023)

I am eligible to work in the United States (an I-9 will be required if accepted into the program)

I understand that a background check will be required (any associated cost will be covered by the program)

I understand that a physical exam and TB test may be required by my host K-12 district

I understand that travel to work site(s) within the K-12 host district will be required

I understand that I will be required to access supplementary instructional materials in a virtual (online) format

I understand that I will be required to obtain a ServSafe Certificate or State/County Food Handler’s Permit by completion of the probationary period (cost will be covered by the program)

I have the ability to lift up to 50 lbs on a regular basis

I have the ability to stand for extended periods of time

I understand that I will be required to show proof of Covid-19 vaccination OR I will adhere to regular Covid-19 testing by my host K-12 district
