Would you like to honor a Rochester Public Schools staff member for making a difference for your child, your family, or the community? Use this form to make a donation in the name of this special principal, teacher, paraprofessional, bus driver, or other RPS staff member.

If you would like to honor multiple staff members, please fill out as many additional forms as necessary, but please make one combined payment via Paypal to help us save on transaction costs. Question 3: Identifying the Donor

Would you like the Honoree to know who made the donation? The email will either say:

+ "The donation was made by an anonymous person."

+ "The donation was made by <fill-in-the-blank>." (Use the "other" option to fill-in-the-blank.)

Examples: "Mike and Carol Brady", "The Brady Bunch", "Marcia's Mom", etc.

Complete the sentence: "The donation was made by..."

Question 4: Purpose

Why are you honoring this individual? Please enter text that finishes this sentence: "This donation was made...."

Examples: "to thank you for your work with Abdullahi", "because you are so thoughtful", "to celebrate your retirement", "in recognition of a job well done", "as a gift during the holiday season", etc. *

Question 5: Personal Message

Would you like to include a personal message to the honoree in your own words? Length may be a few words or several sentences, it's up to you. Your personal note will be included in the congratulatory email that the Staff Member receives.

Examples:
"Thank you so much for all that you have done for our son. He has learned so much and we want you to know how much we appreciate it!"
"Alex Jackson and our whole family would like to thank you for a wonderful experience in 3rd grade!"
"Congratulations on your retirement - may your future be filled with the perfect blend of adventure and relaxation!"

"You did an amazing job last week and your work is appreciated!"

(If left blank, no message will be included.)

Does RPSF have permission to announce this donation on our social media accounts and/or on our website? (Please note that honorees being announced on our website/social media accounts will be solely at RPSF's discretion, pending your permission. Donation amounts will NOT be included unless special arrangements are made between the donor and RPSF.) *
