To have your tuition/fee payments taken by ACH out of your bank account, please complete the following EFT Authorization Form and send a voided check into the office. If your banking information has not changed and is already on file in the office, it is not necessary to attach a voided check. However, we MUST have an authorized signature each year. Payments will be deducted according to the schedule selected in your tuition agreement.
