Summer Band/Choir

Students wanting to be in the Choir for our Masses at St. Henry, or who would be interested in playing their instrument for Mass will have this time to prepare in advance of the school year. All are welcome but especially students who have conflicts with our weekly after school rehearsal, (which will start up again next year), should be a part of this Summer camp.

I, the parent or lawful guardian of (the “child”), give permission for my child to participate in the activity described and release from all liability and indemnify the Archdiocese of Cincinnati, the Archbishop of Cincinnati, both individually and as trustee for the Archdiocese of Cincinnati, and all parishes and schools within the Archdiocese, and their officers, agents, representatives, volunteers, and employees from any and all liability, claims, judgments, cost or expenses, including attorney fees, arising out of any injury or illness incurred by my child while participating in or traveling to or from the activity.

Please note- a Emergency Medical Authorization form will be sent home with your student upon registration for you to sign and return to school prior to participation. *

I acknowledge my registration is not final until my Emergency Medical form and registration payment is returned to the school office.
