I wish to participate in the activities of the Glanbrook Non-Contact Hockey League and hereby assume all risks and hazards incidental to such participation which includes on ice activities, social activities and travel to from all GNHL activities. I also agree to comply with the Playing Rules and Constitution of the GNHL. I hereby waive, release, absolve and agree to hold harmless, the Executive, Members, Participants, Referees and any other persons involved in the operations of the GNHL activities, of any claim rising out of injury, loss, damage or libel to myself. I hereby certify that I have read and fully understand this agreement/contract.

Membership Agreement *

All cheques and contracts MUST be received by the Treasurer by May 31st. Each member MUST provide a cheque for the full membership fee dated for June 1st of the upcoming season. A late fee of $25.00 will be added to contracts submitted after June 1st.Any current member not complying with this rule will not be drafted and will be returned to the waiting list. Members that request an exemption, MUST provide a written letter to the Treasurer explaining their need for the exemption or extension prior to June 1st. NSF cheques will have a $50.00 fee enforced.
