Registration for UUCGL Religious Education

This is a registration and permission form for all Unitarian Universalist Church of Greater Lynn Religious Education programs for children and youth under 18 years of age for the church year 2021-2022. Regarding PreK-6th grade programming: it will be on Sunday mornings at 10am outdoors, with masks required. Please ensure that all members of your family or COVID pod are completely COVID-symptom-free the day of, and that your children are masked and prepared for time in the woods. You may provide a water bottle for your child but otherwise no food or drink will be served. Applying bug spray ahead of time is recommended. Checking for ticks afterwards is strongly recommended and is your responsibility. Note that there are a number of scenarios we are requesting your permission for depending on the age of your children. If we have extreme weather we may decide we need to either host the PreK-6th grade program indoors or remotely. If you grant permission for your child to participate in indoor programming at the bottom of this registration form, we will still check with you on the day of the decision to determine if you still want your child to participate in indoor programming or would prefer a remote option, but it is helpful for us to have a general sense ahead of time. We are planning to offer indoor religious education programming for grades 7-12 with parental permission.

Photo/Video Opt Out: Throughout the year, UUCGL religious education students are photographed and video recorded as they participate in UUCGL activities. The photos and videos are used in UUCGL communications and promotional materials for our church, in our newsletters, and on our website and social media. Unless otherwise indicated below, I give UUCGL permission to use, edit, reproduce, and publish photographs, video, and audio of the children and youth for whom I am the guardian/parent. Please note: Only check the box below if you wish to OPT OUT of this release.

Below is a list of parent/guardian permissions. Please check all that apply. These permissions will determine which activities your child/children can participate in. If you check all the boxes, you will be granting permission for your children to participate in all of the activities listed as determined by their age and program. If there are any limitations on the permissions that you check to grant (for example, for only some of your children or only under certain circumstances), you can include that in the next open-ended question. The Sunday morning PreK-6th grade outdoor religious education programming will be in the woods owned by UUCGL and in the surrounding Ewing woods and 7th-12th graders are invited to help out as well with your permission. There are risks inherent to these outdoor activities including but not limited to tick bites, mosquito bites, poison ivy, exposure to COVID, risks of falls and cuts and broken bones.

I give permission for my child/children of all ages to participate in outdoor religious education on the UUCGL property under adult supervision

I give permission for any religious education teacher to assist my child/children in applying bug spray during UUCGL religious education

I give permission for any religious education teacher to assist my child/children in washing minor cuts and applying antibiotic ointment and a bandaid during UUCGL religious education. I understand I will be contacted immediately for any serious injury or if the bleeding does not stop immediately.

I give my 7th-9th graders permission to participate in indoor religious education programming and Our Whole Lives Sex Education programming in a combined classroom with the 7th-9th graders from First Church in Salem

I give permission for my 10-12 graders to participate in indoor religious education programming and Youth Group activities, and field trips off site both indoors and outdoors

I give permission for my child/children ages PreK-6th grade, and 7th-12th grade if they would like to be helpers, to participate in indoor religious education programming at UUCGL in the event of extreme weather

I give permission for my child/children of all ages to travel throughout the Ewing woods and UUCGL woods under adult supervision during UUCGL religious education programming
