Purpose of this form
If you are a main campus student and will be going out of state or out of country for the summer, and you wish to take online classes while away from main campus, this is the form to complete.

If you are leaving main campus for the summer but will still be in Virginia, you do not need to complete this form. Please register for WC2/WEB2 parts of an online course.
