Building Usage Request Form
Room assignments are made by the church office. Please meet only in your assigned room. If you need to make any changes to your request, please clear it through the church office. Please have a form filled out and submitted 14 days prior to your event. 
All meetings need to conclude in time for participants to vacate the building by 10pm. When you leave, make sure things are in order and the facility is left as you found it. 
If you see any damage, report it to the church office the morning following your meeting.
