This form is for parent(s)/carer(s) to apply for a leave of absence for exceptional circumstances. This form should be submitted at least two weeks prior to the start of the absence being applied for.

Leave of absence during term time is not a right and is authorised at the headteacher's discretion for exceptional circumstances only.
