Below this agreement, please put your first and last name indicating you have read and agree to this information below. By clicking YES, you are legally signing this document. I/We understand and agree to the policies for team membership as outlined in the team membership form. I/We (parent/guardian) hereby give permission for my child(ren) to participate in dance/recreation instruction offered and release all LaChere’s Fab 5 Studio directors, teachers, owners, team moms, and any other person(s) connected with LaChere’s Fab 5 Studio from any and all claims of any kind or nature arising from participation in the above stated dance programs. I/We hereby waive all claims against LaChere’s Fab 5 Studio (and above listed management) for any loss, theft, damages, or injury of any kind that might result in or upon the premises, or at associated recitals, performances, competitions or related gatherings. In case of injury or illness, I give permission for the above child(ren) to be transported to and receive medical treatment at a medical facility, and I guarantee the payment of all expenses incurred for such transportation and treatment. We use pictures of our students in costume and videos from our performances to help advertise for our studio. We post videos and pictures online, on our website, and on social media. This is our biggest way of advertising and ask you for permission to use videos and photos that you/your child may be in to promote our amazing studio. I grant to LaChere's Fab Studio, its representatives and employees the right to take photographs/Videos of me (my child) in connection with the studio. I authorize this studio to use and publish the same in print and/or electronically. *
