All EFSC members are registered with USFS
Membership Application is for the year: July 1, 2021 to June 30, 2022

Applications for membership are accepted by all members of the Board of Directors and by the Membership Chair/Committee.
EFSC membership allows you to:
Test at USFS-sponsored test sessions
•      Compete at USFS-sanctioned events
•      Participate in USFS qualifying events
Represent your club at regional, sectional and national events
Participate in all sanctioned events

Membership Privilege Policy
The EFSC Board of Directors may withdraw or deny Membership privileges at any time the board determines that a prospective member's conduct may be contrary to the best interests of the club, the sport and those who participate in it.

If at any time, any question arises concerning a membership candidate's ability or desire to further the interests of the Club, a majority vote of the Board of Directors shall accept or reject the candidate's application.

A rejected candidate shall be notified in writing within ten days of the Board meeting and vote and receive a full return of fees paid. Six (6) Months after notice of rejection, candidates may reapply for membership.

Further, at any time, EFSC may rescind or revoke a club members’ membership privileges if the boardof directors finds the club member’s actions or behavior contrary to the best interests of the club, the sport and those who participate in it. In the case of a minor club member, the actions of the minor club member’s parent or guardian may also lead to rescission or revocation of the minor club member’s membership.

If the Board of Directors believes that the club member’s actions or behaviors are contrary to the best interests of the club, the sport and the skaters who participate in it, the Board shall notify the member in writing. If the club member is a minor, or the actions or behaviors leading to board action are those of a minor club member’s parent or guardian. Written notice shall be sent to the address on file with the Club and USFS. Notice concerning a minor club member or parent of minor club member shall be sent to a parent or guardian’s address on file with EFSC. If the only address is an email address, that address and email shall be the form of communication.

The board shall give the member, and the parent or guardian if applicable, the opportunity for a hearing within ten (10)days of receipt of the complaint in accordance with the EFSC Club Constitution and By-Laws. This agreement is in addition to, and not in place of, any provision of EFSC’s Conflict Resolution Policy.

By submitting an application for Club Membership, all potential club members, and parents or guardians or minor members, agree to the terms and conditions of this policy.

INJURY WAIVER 2021-2022
WAIVER AND RELEASE NOTICE: THIS IS A LEGALLY BINDING CONTRACT
In consideration of myself (or my minor child) being permitted by Eugene Figure Skating Club to participate in club-sponsored events, I agree to the following: I ACKNOWLEDGE THE INHERENT RISKS ASSOCIATED WITH FIGURE SKATING. I understand that; (a) the sport of figure skating is a hazardous athletic activity; (b) participation in figure skating or related training may result in serious injury including but not limited to permanent disability, paralysis or death; (c) these risks and dangers may be caused by the acts or omissions of the officers, directors, members or agents of Eugene Figure Skating Club, the acts or omissions of other participants, accidents, breaches or contract, or other causes; (d) these risks may arise from foreseeable or unforeseeable causes. I further understand that the club is a community in which open communication takes place. That communication may include the discussion of coaches, officials, skaters, and parents. I agree to be bound by the ethics rules of the US Figure Skating and the Parent’s Code of Conduct. I AGREE TO ASSUME ALL RISKS OF PERSONAL INJURY. In return for being allowed to participate in the skating and non-skating activities of the Eugene Figure Skating Club either myself or for my minor child or ward, I agree to assume all risks foreseen or unforeseen, foreseeable or unforeseeable that arise from participation in the sport of figure skating regardless of the cause except for deliberate and unprovoked physical assaults and the injury or damage that may cause. I accept the financial responsibility for any losses and/or damages, whether caused in whole or in part by the conduct of the officers, directors, members or agents of the Eugene Figure Skating Club. On behalf of myself, my personal representatives and my heirs, hereby voluntarily agree to release, waive, discharge, hold harmless, defend and indemnify Eugene Figure Skating Club, its officers, directors, members and agents, from any and all claims, action or losses for bodily injury, property damage, wrongful death, loss of services, loss of companionship or otherwise which may arise out of my (or my minor child’s) participation in club-sponsored activities. I specifically understand that I am releasing, discharging and waiving any claims or actions that I may have for the negligent acts or other conduct by the officers, directors, members or agents of the Eugene Figure Skating Club. I agree to process any grievance or wrong done by the spoken word, writing in any form or broadcast or publication that is related to skating through the grievance processes of the Club, US Figure Skating or the PSA and to be bound by the result as a final and complete resolution. Further, to the extent that any claim by me or on behalf of my child or ward (arising from my skating activities while a member of the Club) against any third party results in a claim by that party against the Eugene Figure Skating Club for indemnity or contribution, I will save, defend, hold harmless and indemnify the Eugene Figure Skating Club from that third party claim. I also grant to “Club” the absolute right and permission to use, publish, broadcast and copyright any and all material done in connection with “Club” including, Athlete’s voice recording, name, picture and likeness or any material based on or derived there from in any manner or media whatsoever for purposes of advertising in promoting and publicizing “Club” and “Club’s” events. BY MY SIGNATURE BELOW I WARRANT THAT I: 1) Am at least 18 years of age, and am otherwise legally competent to sign this Agreement; 2) Have read the above statement and understand their provisions; 3) Understand the risks associated with participation in figure skating activities; 4) Assume full responsibility for the consequences of choosing to participate; 5) Hereby knowingly and intentionally release and hold harmless the Eugene Figure Skating Club, its officers, directors, members, and agents harmless from any and all claims, demands, injuries, actions (including third party claims), and damage of any kind and nature whatsoever which arise out of, or in, connection with participating in this activity, regardless of whether such claims are based upon negligence or other grounds. 6) Agree to be bound by and abide by the Rules, Regulations, and Bylaws of the Eugene Figure Skating Club and US Figure Skating. 7) Acknowledge that Information on this Application is true and correct and is part of Skater’s US Figure Skating record. * * * ALL PERSONS MUST AGREE TO THIS WAIVER BEFORE GETTING ON ICE TIME SPONSORED BY THE EUGENE FIGURE SKATING CLUB. EFSC CODE OF CONDUCT
EFSC has a zero-tolerance policy for unsportsmanlike behavior:
Members, skaters, coaches, officials, parents and supporters should continually raise the standard of behavior on and off the ice. Membership in EFSC and participation in EFSC hosted activities is a privilege and a source of pride. Program Participation requires that all coaches, members, skaters and parents accept certain responsibilities.

MEMBER/COACH RESPONSIBILITIES:
● I will not verbally or physically abuse any of my fellow members, other USFS members, USFS officials, Club Board Members, Learn to Skate USA program participants, volunteers, or coaches.
● I understand that poor sportsmanship, on or off the ice, will not be tolerated.
● I will continually strive to maintain a positive environment at EFSC.
● I will abide by the EFSC Code of Conduct, Safesport, and the USFS Codes of Conduct and Ethics.
● I will represent myself positively and not undermine the authority of coaches, board
members or officers, officials or other club volunteers.
● I will read and follow the Bylaws of EFSC, and all other club policies and directives.

PARENT/GUARDIAN RESPONSIBILITIES:
● I will model and encourage sportsmanship by showing positive support for all skaters, members, coaches, board members, club officers, and other officials both on and off the ice.
● I will encourage my child to treat all other skaters, members, coaches, officials, board members, club officers and other member’s parents with respect.
● I will refrain from coaching my child or directing other skaters during any on- and off-ice sessions (excluding parent volunteers in assigned duties or guest coaches at EFSC).
● I will not verbally, mentally, or physically abuse any club members (including my own child), skaters, coaches, club officers, officials, or other club volunteers.
● I will represent myself positively and not undermine the authority of coaches, club board members or officers, officials or other club volunteers or act in a way that diminishes or demeans EFSC or the programs it provides.
● I will also read and follow the Bylaws of EFSC, and all other club policies and directives.

COURSE OF ACTION:

Should a parent, coach, skater or member not adhere to the above Code of Conduct, EFSC holds the member, coach, skater and/or parent accountable and the Board is empowered to take needed action. Actions may range from a simple verbal or written warning up to suspension or expulsion from EFSC. All Board of Directors decisions are binding and final.

EFSC SafeSport Statement for Membership Renewal

Eugene Figure Skating Club is committed to creating a safe and positive environment for members’ physical, emotional and social development and ensuring that it promotes an environment free of misconduct.

The following code of conduct applies to all participants in activities hosted, supported, sponsored or engaged in by U.S. Figure Skating and (Club/Program/Rink) including but not limited to competitions, exhibitions, training camps and local rink activities.

I recognize that my participation or my child's in all the activities associated with U.S. Figure Skating and (Club/Program/Rink) is an honor and privilege that carries certain responsibilities. I agree to fully abide by the rules and guidelines set forth by U.S. Figure Skating or its properly designated agents.

U.S. Figure Skating Member Code of Conduct GR 1.02

I recognize that my participation in all activities hosted, supported, sponsored or engaged in by U.S. Figure Skating, including but not limited to competitions, exhibitions and training camps, is an honor and privilege that carries certain responsibilities. I agree to fully abide by the rules and guidelines set forth by U.S. Figure Skating or its properly designated agents. As a precondition to participation in activities hosted, supported, sponsored or engaged in by U.S. Figure Skating, I will adhere to the following tenets in good faith:
A. I will exhibit the highest standards of fairness, ethical behavior and genuine good sportsmanship in all of my relations with others.
B. I will not damage public or private property. I understand that I may be held financially responsible for damage deemed to be wantonly or willfully executed on my part, and that I may be subject to disciplinary action by U.S. Figure Skating.
C. I will not use or possess illegal drugs, and I will not engage in criminal activity. I understand that, if I am found to use or possess illegal drugs, or if I am found to engage in any criminal activity during any activity hosted, supported, sponsored or engaged in by U.S. Figure Skating, I may be subject to criminal penalties as well as penalties imposed by U.S. Figure Skating.
D. I will adhere to the rules of U.S. Figure Skating and the host organization at all activities hosted, supported, sponsored or engaged in by U.S. Figure Skating.
E. I will comply with all applicable anti-doping rules including, but not limited to, ISU and USADA anti-doping rules.
F. I will conduct myself in a manner not detrimental to the welfare of figure skating. I understand that my actions reflect on U.S. Figure Skating and the sport of figure skating both positively and negatively. I understand that if my acts, statements, or conduct are considered detrimental to the welfare of figure skating by the appropriate authority, I may be subject to penalties imposed by U.S. Figure Skating pursuant to GR 1.04.
G. I understand that the penalties that may be imposed may include, but are not limited to, loss of future international selections, loss of financial support from U.S. Figure Skating and its Memorial Fund, and loss of participation in activities hosted, supported, sponsored or engaged in by U.S. Figure Skating.
H. I understand that all disciplinary proceedings will be conducted pursuant to Article XXV, Section 3, of the U.S. Figure Skating bylaws, and that my rights and remedies are derived therefrom.

COVID-19 WAIVER FORM 2021-2022
In consideration of being allowed to participate in any EFSC program-related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. Participation in EFSC programs and activities includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, SARS and COVID-19. While particular rules and protocols as well as personal discipline may reduce this risk, the risk of serious illness and death does exist; and, 2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation and compliance; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation as it regards to protection against infectious diseases, including following any and all protocols for handwashing, sanitization, mask-wearing, physical-distancing, and other safety-related protocols put in place. These terms and conditions include but are not limited to policies put forth by the EFSC, the facilities management, as well as state and local guidelines, ordinances, and mandates; and,
4. I accept that if I am unwilling to comply with such terms and conditions, I will not be allowed to participate in EFSC events and activities and may be asked to leave the event/activity facilities and will not be refunded any fees incurred for such events/activities; and, 5. If I observe any unusual or significant hazard(s) during my presence or participation, I will remove myself from participation and bring such matters to the attention of the nearest official immediately; and, 6. I agree that if I have or am beginning to experience fever, a new or worsening cough, or shortness of breath that I will voluntarily not initiate or continue participation in any EFSC programs, practices, or activities for a period of 72 hours following complete resolution of those symptoms. 7. I agree that if I have been in contact with anyone that has been diagnosed with Covid-19, I will not participate in programs, practices, or activities for a period of 14 days from the date of exposure; 8. I agree to monitor my temperature and inform the EFSC if I have a positive Covid-19 diagnosis; and, 9. I, for myself and on behalf of my heirs, assignees, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS AND WAIVE ANY AND ALL CLAIMS related to contracting or being exposed to an infectious disease including but not limited to MRSA, influenza, SARS and COVID-19. I hereby release and hold harmless the EFSC, its officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), with respect to any and all illness, disability, death, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE related to contracting or being exposed to an infectious disease including but not limited to MRSA, influenza, SARS and COVID-19, to the fullest extent permitted by law.
