Please submit this form along with the $50 Enrichment Club fee. Your child will not be enrolled without a completed form AND paid registration fee. Registration fee should be turned in to the Freedom Intermediate Front office in an envelope with your child's name and Gentry's Enrichment Registration Fee c/o Miss Tisdale.
