Please complete this form if you are interested in volunteering as a chaperone for the 6th grade field trip this Tuesday, November 15th.  Our 6th grade students will be visiting the water treatment plant as well as completing activities at Johnson Park.  Volunteers will need to get a visitor's badge from the front office, then meet the group at the assigned location.  Parents/guardians are not permitted to ride the bus with students.  You will be contacted by a 6th grade teacher to confirm your request and assign you a responsibility.
