To apply for the award complete the following steps:

Online Submission: Complete this online application form. Submit the application by clicking the “SUBMIT” button at the bottom of the form. You will automatically receive a message confirming the form has been submitted. To pre-check your total score and keep a copy of the application for your records, the 21st Century School Library Self-Assessment can be used as a worksheet/tool. Access this tool at

2.
SUPPORTING EVIDENCE & EXPLANATION: In an email to the School Library Technology Coordinator, attach at least one form of evidence and a supporting explanation in EACH of the following areas: Place, Professional, and Program. There should be no less than a total of three forms of evidence submitted (at least one for each area). All submissions must be in digital format. Evidence includes but is not limited to video, photo, link, website, lesson plan, staff presentation, programming, etc. The supporting explanation, describing the connection to the evidence, may be incorporated into the evidence using a tool such as a slideshow, document, web tool, etc. You can also record a video explaining your evidence. You will receive a confirmation email upon submission.
