Math Circles Registration Form

This form is used to register for the Fields Math Circles program, running on Saturdays from January 13th until June 22nd. Please note that filling in this form does not guarantee you a spot in the program as space is limited. Spots are filled on a first come, first serve basis and those who do not get a spot in the first round will be placed on our wait list.

You will receive a confirmation email after you register letting you know your status in the program and how to join us. If you are on the wait list and space becomes available we will send you an email.

Information shared on this form is covered under the Fields Institute Privacy Policy (see below) and will only be shared with members of the Fields Institute including the volunteer coordinator for this program.

This is a free program with limited space. By submitting this form you are agreeing to attend the program every Saturday. If you are unable to attend you will let our coordinator know at least 24 hours in advance. Failure to do so on a regular basis will result in your removal from the program so that your spot can be given to someone on the wait list. By submitting this form you are agreeing to these terms.

We may disclose personal user information to employees of the Fields Institute. All employees and contractors at the Fields Institute are bound by non-disclosure and confidentiality agreements.

We will not share or sell your data with other third-parties, except in the following cases:

1. With the explicit, written consent of the individual to whom the information pertains;

2. For purpose(s) for which the personal information was collected (see above);

3. As part of an anonymized data set provided to a third party upon request for information about the characteristics of the Fields Institute users;

4. To a Fields Institute employee, Director, agent or contractor who needs the information to perform official, proper duties on behalf of the Fields Institute, and who is bound by non-disclosure agreements regarding use of any data collected by the Fields Institute;

5. Externally only as required by law or a legal proceeding.
