進場說明

*任何人士必須在進入比賽場地前提交健康申報表，否則將被禁止進入比賽場地。
All visitors must submit a health declaration form before entering the match venue, otherwise they will not be allowed to enter to the match venue.

** 已填寫網上健康申報表人士，於進場前必需出示回覆的郵件予大會工作人員核實資料方可進入比賽場地。
Every person who have already filled in the online health declaration form is requested to provide the contact number to verify the information before entering the match venue.

***進入場地均受政府防疫條例規範，場地擁有最後決定權是否准許申請人進入有關場地。

Entry into the venue is regulated by the government's epidemic prevention regulations, and the venue has the final decision on whether to allow the applicant to enter the relevant venue.

聲明及注意事項 Declaration and Notice:

本人聲明據本人所知及所信，以上資料均屬正確無誤，並同意大會根據其資料保密守則處理及使用本人所提供資料。
I confirm that the above information is accurate to my best knowledge and agree that such information will be processed and used in accordance with the HKFA’s Code of Confidentiality of Information (Code of Conduct Paragraph 18).

所收集的資料僅為確保安全及健康，有關資料將於到訪日期起180天後銷毀。
The information collected will only be used to ensure the safety and health and will be destroyed from online storage after the date of visit 180 days.

訪客進入場地後需遵守場地防疫規定及遵從大會安排，如因任何原因受傷、身故、染疫或財物損失，均不會向主辦、協辦、贊助、資助等單位索償。

In the venue, visitor(s)/applicant(s) must abide by the venue's epidemic prevention regulations and the organizer's arrangements. In case of injury, death, infection or property loss for any reason, visitor(s)/applicant(s) will not claim compensation from the organizer, co-organizer, sponsor, sponsor, etc.
