HEALTHCARE APPOINTMENTS, a
DOCTOR'S NOTE IS REQUIRED WITHIN 2 DAYS upon your student's return to school.
-Late notes/notifications will not be accepted per district guidelines.
-A doctor's note is required if your student is out for more than 3 consecutive days.
-Quarantining is not required because of parents or family members who test positive.
Form must be filled out for each day that the student is absent from school
