Please complete this form to receive a Cleveland Hill Elementary Parent Pick-Up car tag.  This tag must be hanging from your rear view mirror with the numbers visible when you enter the ES Car Lane.  Car Tags must be returned to the Elementary School Main Office upon leaving Cleveland Hill Elementary, either through moving into Cleveland Hill Middle School or transferring outside of the Cleveland Hill School District.
