This is the SSP Pre-Enrollment Form - Wait List, you will be notified if your child/children are moved from the wait list and put into the Summer Program. *If you are enrolling at Folsom Elementary there are currently spots still available. 

The 2024 Summer Program will be held at Hellstern Middle School in Springdale and Folsom Elementary School in Farmington, for grades K - 6th, from June 3 through August 2, 2024. Your child's grade will be determined by the grade they are entering for the 2024/25 school year. The hours of operation are 7a - 6p, Monday thru Friday.

$75 registration fee per child
1 child: $180/week tuition
2 children: $350/week tuition
3 children: $500/week tuition
4 children: $600/week tuition
I understand that the 2024 Summer Program is only accepting full time enrollment, and that I will be responsible for the full rate of tuition from June 3 - August 2, 2024. *

I understand that completion of this pre-enrollment form does NOT guarantee my child's(children's) enrollment in the Summer Program. Pre-Enrollment forms for summer will be accepted through March 15, 2024. I will be notified by March 29, 2024, if my child(children) is(are) accepted.
