Safety First Partnership Agreement & Assumption of Risk (beginning summer 2021)
The Safety First Partnership Agreement is between Frequent Flyers Aerial Dance
and the participant filled in at the bottom of the form and their parent(s)/legal guardian(s).  
Each time you enter the Frequent Flyers Aerial Dance studios, you agree to the following:

Have not tested positive for COVID-19 or experienced the following symptoms (of unknown cause) in the past 10 days, including: Cough, Shortness of breath/difficulty breathing, Chest tightness, Fever, Chills, Repeated shaking with chills, Muscle pain, Body aches, Fatigue, Sore throat, New loss of taste or smell. *
Have not been in contact with anyone who has tested positive for COVID-19 or the experienced the above symptoms (of unknown cause) in the past 10 days. Or if I have, I am fully vaccinated and do not need to quarantine according to CDC guidelines. *
Have read and understand the facility's safety procedures posted at https://frequentflyers.org/covid-19-policies/ to reduce the risk of spreading of COVID-19,  and I agree to follow those policies. *
