The proposer has submitted, or intends to submit, a response to the RFP that will list you as a reference. In their request to you, they should have provided a Reference Request Form with a background summary as well as instructions for this process.

Please complete this Reference Questionnaire and submit by or before the proposal or statement of qualification submission deadline. Complete one questionnaire per individual; e.g., if you received five (5) Reference Request Forms from the proposer, you would complete five (5) reference questionnaires, even if such individuals all worked at the same firm, on the same project. Provide as much detail as possible.
