An athlete may be driven to and from games with prior notice to the Supervisor of Athletics. Their parent or guardian, must complete the following permission form so the athlete may be transported to or from a contest at least one (1) day in advance of the event. Athletes are not allowed to be transported to or from any athletic contest by anyone other than their parent or guardian. Any athlete that leaves an away athletic event without prior written approval of the Supervisor of Athletics as outlined above, will face disciplinary action including, but not limited to, suspension from the next athletic contest.
