We are all challenged by Covid 19.

The following are guidelines to be followed. During this time of elevated case numbers, strict adherence to the guidelines are absolutely mandatory to manage risk of infection with COVID 19.  People who are at risk according to CDC guidelines, or are uncomfortable attending gatherings should consult a physician to make a decision on attendance.  

Please acknowledge for all people you are registering.

-I do not have a fever. I feel healthy, and I have had no flu-like symptoms or other symptoms consistent with COVID-19 infection for at least seven days.
-I have not had any loss of the senses of smell or taste in the past seven days.
-I do not have shortness of breath, cough, runny nose, congestion, or other respiratory symptoms and haven't in the past seven days.
-I have not had close or sustained contact with anyone with the above symptoms in the past 14 days
-I understand that I MUST wear a mask covering my mouth and nose.
-I understand that I MUST maintain a minimum of 6 ft. distancing and 12 ft. minimum distancing for events involving projected voices (I will maintain this proper distance from anyone outside of my family unit submitted on the registration, regardless of vaccination status).
-I understand that I MUST bring my own Siddurim and Kipot (if applicable), Talit (when needed). I may contact the office to purchase or borrow Siddurim.
-I understand that no food or drink will be provided by Temple Beth Sholom.

-I understand that if I do not follow the practices listed above that I will be asked to leave the premises immediately without question.

The address of the temple is 3400 North Dobson Road, Chandler, AZ 85224. Plenty of parking.
