Mission Cognition On Site Social Skills Group Application

Please thoroughly read & review the following prior to submitting the application: 

This application is the first step in the intake and enrollment process for social skills groups at Mission Cognition Social-Emotional, Behavioral Wellness Program. 

The information obtained on this initial application helps the team determine whether or not your child would be a good candidate for the program at this time.


General Overview of Philosophy & Service Delivery Model

  • Location is Hillsborough, NJ. Groups are on site, in person only at this time.  As of January 1, 2024, Mission Cognition has relocated to 308 Route 206

  • We offer groups for preschoolers- high schoolers with a range of diagnosis and social needs across differentiated Level A, B, & C groups. 

  • Best fit groups are formed according to participant age as well as areas of strength, need, interests and preferences. All of our groups are highly individualized to best meet the needs of the participants in the group. 

  • The structure and routine is customized to focus on 3-5 priority global focus areas (social language & social behavior domains) which have been identified as a priority during the assessment process. Additionally, each activity is structured and facilitated in a way which embeds each participant’s individual goals with ample opportunities for practice and generalization across social coaches, peers, activities and materials.

  • Sessions run seasonally Fall, Winter & Spring with an abbreviated summer model.
    New participants are not accepted during the summer without a Fall continuation commitment. Participants enrolled in the spring may take a summer break without losing their spot for continuation in the Fall. 

  • We are an active and experiential learning based program with an emphasis on following participant motivation, increasing reciprocity and engagement with a focus on teaching skills through play, leisure, practical life & movement. Research supports that teaching in environments, through situations and with materials which closely simulate the natural environment increases likelihood of generalization which means acquiring skills which are used where it really counts (recess, birthday parties at home with family, on vacation, at work, visiting friends, etc!)

Intake, Assessment & Enrollment 

  • Upon submission of this application, you will receive an automated confirmation email with a link  to schedule your complimentary discovery call. The purpose of the introductory call is to learn more about your child's specific areas of strength and need, as well as further detail the service delivery model and answer any general questions you may have about the logistics of the program. 

  • If it is mutually decided to move forward with the intake and assessment process for placement in a best fit group, there is a $125 assessment fee which includes parent/caregiver interview, participant interview (if applicable), the review of submitted rating scales & checklists, review of any provided outside evaluations & reports including: IEP, BIP, Speech, OT, ABA as well as structured observation and direct assessment to be completed in the first day of group. A complete assessment report with goals and relationship development and skill acquisition plan is provided. This is a one time fee assessed for brand new participant's prior to their first session at Mission Cognition.

  • Mission Cognition is private pay only and does not work with insurance companies. We do not provide CPT codes. If your child is approved for services through PerformCare/CMO, your case manager may approve partial or full payment of services using state funds. It is your responsibility to connect with your care manager to get approval once Mission Cognition has formally offered placement.

  • Session tuition breaks down to approx $65-75 per week &  is inclusive of weekly 90 min sessions, detailed weekly session notes and activity plans, weekly data on goals, one group parent training session via zoom, mid session and final session progress updates and recommendations. Tuition installments are due monthly.

  • Consistent weekly attendance is required to gain the full benefit of the program. If there are more than 2 consecutive planned absences in a given session (12wks), or 3 planned  non consecutive absences, the child is not eligible for enrollment in that session. Three or more absences not due to illness/emergency in a given session may make a child ineligible for enrollment in future sessions. 

  • It is recommended that participants enroll on an ongoing basis for 12-18 months, or until discharge if appropriate sooner, to work through each individualized priority skill acquisition program and have opportunities to track generalization and maintenance across leveled groups.
