Parent/ Guardian Release:
I have registered the student on this form and my signature below indicates that I release and hold harmless the Lee County School District, Trafalgar Middle School and any persons connected with the said programs from liability or responsibility in case of accident or injury to my student(s)person or property incurred during the operations of this program, which is not the direct result of willful action or culpable negligence by the School District or its employees. This includes any and all related travel by a Lee County School bus. There is no medical coverage included in this registration for any program offered by Trafalgar Middle School. By participating in these programs, I assume medical insurance responsibilities and I accept full responsibility for all medical costs in the event of an accident or medical emergency. I understand that in the case of a medical emergency, 911 may be called and I hereby authorize the doctors at the designated hospital to treat my student for any injury or illness occurring during Trafalgar MIddle School's programs.

Refunds:
I understand that there are no refunds for any student. All monies credited to your account must be used toward childcare services. This money will not be refunded to students who leave during the school year nor will this money be refunded to eighth-grade students who are leaving the building.

Pick up and Drop off:
6:30 AM drop off.

6:00 PM pick up.

Payments:
The State of Florida Constitution Article 17 states that governmental agencies cannot extend credit therefore, payments must be made in full on a weekly basis. If fees are not paid, it may result in your child being suspended from the program. Payment can be paid in cash or check (made payable to Trafalgar Middle) Monday – Friday. I understand that the cost of the Before School program is $35.00 per week or $7 per day and After School program is $35.00 per week or $7 per day. ($70.00 per week for both programs or $14 per day for both programs). There is a non-refundable registration fee of $25.00 per child. Payments must be made by the Monday of the week my child attends. Students will be suspended from the program if full payment is not made on Monday. We are only responsible for payments made directly to us in the school office or Program Staff member. We are not responsible if payment is given to a teacher, hand-delivered to the office, or any other school personnel.

Our enrollment may be limited. Our entire program, grade levels, or special needs classes may be closed to new enrollment at any time during the year.
