We, the undersigned, hereby makes application to the Camdenton R-III School District for the use of school premises and certifies that the information given above is correct.  The applicant will exercise the utmost care in the use of school premises and property and will be held responsible should any damage occur from the applicant's use of said premises and property.  We hereby agree to defend, indemnify and hold harmless the Camdenton R-III School District, Board Members, Administrators, Teachers, Employees, Volunteers, and Agents from and against any/all liabilities, claims, damages, penalties, actions, suits, losses, costs, and expenses arising out of connection with the use of said premises.  This agreement to defend, indemnify and hold harmless specifically includes all claims arising out of any accident or other occurrence on or about said premises causing injury to any person or property.  The undersigned has read and is in agreement with the rules/regulations for the building/facility.
