Please complete this electronic registration and consent form. It will provide us with information to help us plan for the testing regime which we hope to begin for all students and staff after when school reopens after the National Lockdown. Please not we will be testing staff and students that are in school for onsite learning.

The proposal to test students and staff in school has been welcomed and will be of benefit to help us to keep the school open, enabling students to remain in lessons and staff to teach. It will be an addition to our existing stringent safety measures to ensure our school is as safe as possible for everyone in our community.

Although testing is NOT COMPULSORY and we require parental consent, we strongly encourage all students and staff to participate in this process.
