Getting things wrong provides a great amount of data you may choose to learn from.  It is important you communicate, BY FILLING OUT THIS FORM, what might have gone wrong the first time.  This form will notify me you have updated your portfolio so I can check it AND update your grade in power school, if you have provided a valid reason to try again. You MUST ALSO add your rework form submission to your portfolio!  I will not accept reworks if you have not included your submission receipt in your portfolio.  **Late points may apply**
