Weekly form needed by school leadership for families seeking on-site attendance for their child(ren).

The Victorian Government has stated that all students who *can* learn from home *must* learn from home.

You may be contacted by school staff to ascertain on what basis you are requesting the on-site attendance, to ensure that Victorian Government guidelines are being followed.
