2022 CBA Students Summer Camp 6/6-8/14/2022

We are inviting you to do this online registration, to select the best time for your child's summer badminton camp.
Summer gives kids free time away from school, and your children have time to do some fun, healthy, physical, skillful indoor badminton activity.

CBA Summer camp has a 10-week camps in weekly basis.
* 7 days per week options (Mon--Sun)
Camp Schedule: Mon - Fri (Session 1, 2, 3, and 4); Sat - Sun (Session 5, 6)

Summer Camp coach:
CBA offers high quality Badminton professionals experienced with China & Malaysia National team members to be your kids’ coach. We will be making sure your kids feel fun, happy, and learn real Badminton techniques in our Camp.

How to sign up & Summer Camp payment require:

1. Fill out accurate info which required.
2. If sign up two or more kids, please sign them up separately.
3. Sign up your selected day with our regulation.
4. Tuition count by each session $45/per session,
5. Please process the camp payment within 5 days after your register CBA Camp and your spot will be secured.
   otherwise, CBA can not promise your registration.
6. Pay by check at CBA front desk, or Zelle payment must be indicated Name, camp schedule.
7. Sibling discount $10.00 for 2nd Child one time only.
8. No Make-up Class and Change Schedule when payment is processed.
9. No refund once your payment is processed.

Camp Selection：
1.  1 session/ day/ week/  must sign in 3 camps
2.  2 session/ week/ must sign in 3 camps
3.  3 session/ week/ must sign in 2 Camps

Camp Policy:  
1.  Camp T-shirt and Badminton shoes required for each class
2.  Water only, No food and drinks in the camp

Waiver Agreement:          Waiver and Release California Badminton Academy (CBA) is committed to conducting its programs and activities in the safest manner possible and holds the safety of participants in the highest possible regard. Participants and parents registering their children in programs and activities must recognize, however, that there is an inherent risk of injury when choosing to participate. CBA strives to reduce such risks and insists that all participants follow safety rules and instructions which have been designed to protect the participant's safety. Release of Liability & Permission to Secure Treatment I recognize and acknowledge that there are certain risks of physical injury to participants in the above program(s) and I agree to assume the full risk of any injuries, damages or loss regardless of severity which I or my minor child/ward may sustain as a result of participating in any and all activities connected with or associated with such program(s). I agree to waive and relinquish all claims I or my minor child/ward may have against California Badminton Academy and its officers, volunteers and employees as a result of participation in the program. I do hereby fully release and discharge California Badminton Academy and its agents, officers, volunteers and employees from any and all claims from injury, damage or loss with the activities of the program(s). I further agree to indemnify and hold harmless and defend the California Badminton Academy and its officers, agents, servants and employees from any and all claims resulting from injuries, damages, and losses sustained by me or my minor child arising out of, connected with, or in any way associated with the activities of the program(s). In the event of any emergency, I hereby authorize California Badminton Academy to secure from any licensed hospital, physician and/or medical personnel any treatment deemed necessary for me or my minor child/ward’s immediate care and agree that I will be responsible for payment of any and all medical services rendered. Photography Release I give permission for my child’s picture to be used in media for California Badminton Academy. I have read and fully understand the above Release of Liability and Permission to Secure Treatment and Photography Release. *
