FMA Uniform Order Form
The free FMA Uniform Store (located in the hallway behind the gym) was launched in 2019. It has numerous gently used clothing (in all sizes) that comply with the FMA uniform guidelines.

Since this year families can't go pick-up clothes themselves, families can order clothes to be delivered to their child's classroom. Please feel free to order as many items as needed, there are plenty of clothes!

If you have more than one child, please complete a separate order for each child.
