Please fill out this Google Form as soon as you know about a schedule conflict. Filling out this form DOES NOT excuse the student from the rehearsal or performance. This form only notifies Mr. Ma of the schedule conflict and documents the conflict. Excusal and any potential make up works will be determined and assigned by Mr. Ma after he has been notified. Please make sure you notify him at school so he can determine how to move forward with your notification.
