In lieu of sending in paper notes to teachers for pick up, please complete this form if your child has a change in their dismissal for the day. This form MUST be completed prior to 2:00 p.m. to be included on the 3:15 p.m. dismissal list. If you are requesting your child be picked up as an every day pick up (or every Monday, etc.) please email Mrs. Feldman the day(s) of the week that your child will be picked up.
