By signing this form, I, the undersigned participant (or parent/guardian of the participant), authorize participation in the Youth Night at the aforementioned mosque. I acknowledge and agree to the following terms:
Assumption of Risk: I understand that participation in Youth Night involves inherent risks, including but not limited to personal injury, property damage, and loss of personal belongings. I voluntarily assume all risks associated with participation, whether known or unknown.
Behavior and Conduct: I agree that the participant will abide by all rules and regulations set by the mosque for the duration of the event. This includes respecting the property, other participants, and organizers.
Waiver and Release: I hereby release, waive, discharge, and covenant not to sue the mosque, its trustees, officers, employees, volunteers, and agents from any and all liabilities, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant, or to any property belonging to the participant, while participating in Youth Night, or while on the premises where the event is being conducted.
Medical Treatment: In the event of an emergency, I give permission for the organizers to secure from any accredited hospital, clinic, and/or physician any treatment deemed necessary for the participant’s immediate care. I understand that I will be responsible for any expenses incurred for such treatment.
Photography/Video Consent: I consent to the use of photographs and videos taken during the event for the mosque’s newsletters, website, social media, or other promotional materials.
Understanding of Terms: I acknowledge that I have been given ample opportunity to read and understand the terms of this waiver and release. I understand that by signing this form, I am waiving legal rights and remedies that may be available to me or the participant.
