Complete this form and turn in payment to BCAPA. T shirts will be black, short sleeved shirts with a black and white BCAPA logo on the front. Each shirt is $25 each and sizes range from youth xs through adult 4xl. Payments can be made by cash (be sure to label the envelope as a t shirt payment along with your name), by checks made out to BCAPA, or with a credit card through studio director (located in the online store tab). Cash and check payments should be placed in the BCAPA drop box in the lobby.
There are spaces for up to 4 shirts on this form. If you would like to order more than 4 shirts then you will need to fill out an additional form for the remaining shirts in your order. Orders are not considered complete until payment is received.
