MVP Contact Information and Questionnaire
The Spring 36 Music Video Program (MVP) will take place ONLINE SATURDAYS, July 10 - August 28th.  If you live in the same area as other participants, and would like to participate with others while outdoors and following COVID-19 Safety protocols, that is up to your guardian(s).
All participants will also be able to sign up for at least 1 private lesson and 2 small group lessons in the areas of their choice.  These lessons will take place before or after the group lesson times.  Limited weekday options will be available for the private lessons.   Tuition for the entire program is $545 per participant.  After filling out this form, participants will receive participation waivers and additional information on how to submit payment.  Thank you, and please email Spring36LLC@gmail.com with any questions!

CAPEZIO has made a generous product donation which includes a variety of sizes and styles.   Please let us know if you are in need of a particular type of dance shoe or apparel, and we will do the best we can to match requests with the products we received.   If you already have dance shoes, but perhaps have a sibling who could use dance shoes, you may mention that, and if we have enough, we will send them as well! *

During this program, participants will be taught elements of video production, recording, and editing.  Please confirm if you have any equipment such as a video camera, phone with a recording device, portable speaker, headphones, editing software, etc.  If you do have any of these items, please list them.
Private Lessons and Additional Small Group Lessons.  Participants will have the opportunity to take at least one private lesson and two small group lessons in addition to the regularly scheduled programming.  Please select the time ranges when you are generally available for these bonus classes.  For example, if Brandy is a student and her main interests are Editing, Nutrition, and Auditioning Skills, and she is available on Saturdays from 10:30 - 12 and 3:30 - 5p.m. all Saturdays, she may receive a bonus schedule such as this: July 24th, 10:30a.m. - 10:55a.m. Editing with Mike, July 31st, 11:30 - 11:55a.m. Nutrition with Rob, and August 14th 4:40 - 5:05 Auditioning Skills will Tempestt.  Please select all available times.
