As per section IV, Article 4.2.1 of the ICEC Policies and Bylaws, the following information is being requested in order to maintain active chapter status:  Submit this completed report by September 30th annually.  As a student chapter your officers may change during the year (after this form is submitted) please just change that contact information when needed.
