Revolution FC 2020-2021 Season Sign Up Registration
To register for Revolution FC competitive Soccer Tryout for the 2020-2021 season, please complete this form and submit by Noon of the day of the Tryout.

Players should bring their own ball and water to kick around. Please be prepared upon arrival to the fields by having all soccer cleats, socks, and shin guards on before taking the field. Please check website prior to date for any updates on time, place, dates, health guidelines.

NO PARENTS WILL BE ALLOWED AT THE FIELDS. We ask parents remain in their vehicle in accordance to the guidelines set forth by MYSA. Parents are permitted only to the registration table for player check in and questions. Players will be asked to keep their belongings at a distance from others and to use hand sanitizer before and after training. All club equipment will be sanitized and thoroughly cleaned before and after each session. Parents are asked to take players temperature prior to coming to practice.

By submitting this form I, the parent/guardian of the individual named below, do hereby agree to indemnify and hold Revolution FC, their coaches, staff and volunteers from and against all liability resulting from participation in competitive soccer try outs. I understand that soccer has inherent risk, for which I agree to assume complete liability. Furthermore, the player listed below is in good physical health for the activities which they will be participating. In the event that my child requires medical attention and I cannot be reached, I authorize Revolution FC to obtain medical treatment for my son/daughter with no liability to the cost incurred. I agree to pay all medical cost that incurred for my son/daughter or legal ward. Revolution FC is taking all precautions and guidelines set forth by both MYSA and our state and local health department to help protect all our players and their families from the spread of COVID-19. I understand the risk involved with allowing my child to participate. If my child or a member of our family contracts COVID-19, I do hereby agree to indemnify and hold Revolution FC, their coaches, staff and volunteers from and against all liability. *
