COVID-19 Liability Waiver: By submitting a Coalition of Families for Inclusion (COFFI) event registration form, you attest that you are the parent or legal guardian of the minor(s) named in the registration. You acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that you or your child(ren) may be exposed to or infected by COVID-19 by attending any COFFI sponsored event and that such exposure or infection may result in personal injury, illness, permanent disability, or death. You understand the risk that you or your child(ren) becoming exposed to or infected by COVID-19 at COFFI sponsored events may result from the actions, omissions, or negligence of myself or others, including, but not limited to, COFFI volunteers, program participants and their families.
 
You voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to yourself and your child(ren) (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that you or your child(ren) may experience or incur in connection with you or your child(ren)’s attendance at COFFI sponsored events (collectively “Claims”). You also understand that if the COFFI is not able to host in-person events in order to comply with state and local guidelines, the COFFI Board committee will make every effort to facilitate virtual events. On behalf of yourself and your child(ren), you hereby release, covenant not to sue, discharge, and hold harmless the COFFI and its volunteers, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. You understand and agree that this release includes any Claims based on the actions, omissions, or negligence of COFFI and its volunteers, whether a COVID-19 infection occurs before, during, or after participation in any COFFI sponsored program.

Disclaimer of Liability:  By submitting a COFFI event registration form, you are acknowledging that you and your family members' attendance and participation in any activities organized by the Coalition of Families for Inclusion, including attendance of activities or events at the home of another COFFI member, is your sole risk and responsibility.  COFFI, which collectively includes the organization, its board members, volunteers, members and representatives, is not responsible for any injury, loss or damage of any kind whatsoever sustained by any person or their property while participating in events or activities associated with COFFI.
 
Permission to Use Photographs:  By submitting a COFFI event registration form, you are also granting COFFI the right to take photographs of you and your family, including your children, during events associated with COFFI, and to use those photographs for purposes of communicating about or publicizing the COFFI and/or activities and events associated with us, including through newspaper articles, brochures, newsletters, social media outlets, and other literature regarding the organization.
