UP Baguio HSO - Health Declaration Checklist Form v1.01

For the health and safety of the UPB employees and visitors entering the campus, a declaration of the present health status is required.  Please be accurate and truthful in providing information in this form.  Your contact details are required  to facilitate contact tracing as deemed necessary.  Seek immediate medical advice if you have any Influenza-like signs and symptoms (fever, colds, cough, body pains, sore throat, loss of taste/smell).

Please accomplish this form no more than 12 hours before entering the UP Baguio Campus.
