This application form is required to participate in the Rexdale Volleyball Program operated by Change Through Sport. It must be completed by the participant or by a parent/guardian if the participant is under the age of 18.
This year, the program will host sessions twice per month - the first and third Sunday of every month from Oct 15, 2023 to Jun 2, 2024
